Terms and Conditions of „MOTYL – 2014” Award

I. Organizers and goals

  1. The organizer and founder of the Award is the European Association for Health Promotion"Pro -Salutem" and its partners. Organizerd etermines conditions for obtaining the Award and appoints the Jury, which is responsible for the proper Winner selection process.
  2. The aim of the Award is to promote healthy attitudes and supporting positive changes in the field of gynecology, sexology and obstetrics in Poland simultaneously raising the quality of medical care.
  3. The prize is awarded in four categories:
  1. Motyl 2014 – in category „My Gynecologist and Sexologist” – prize awarded by Internet users voting
  2. Motyl 2014 – in category „Outstanding Gynecologist – Specialist” – prize awarded by the established Jury
  3. Motyl 2014 – in category „The Humanist of Health and Medicine” – prize awarded by the established Jury
  4. Motyl 2014 – in category „Penmanship in the field of gynecology and sexology” – prize awarded by the established Jury

II. Criteria for awarding the Prize – MOTYL – 2014

  1. The award may be granted to individuals that in the last year can be proud of: inventing a new diagnostic or therapeutic method in the field of gynecology, sexology and obstetrics; development of a new strategy relating to the organization and management in the field of gynecology, obstetrics sexology; modernization ofthe Polish health care system through implementation of innovative global solutions; original scientific achievements and innovative implementing solutions in the field of clinical or experimental gynaecology; humanistic and writing achievements in aforementioned fields.
  2. The right to nominate the Laureates belongs to third parties (in particular employers, managers of units in which Award candidates are employed or carry out research, social and professional organizations) on condition of getting the written consent for participation in the contest from the particular innovation author and Internet Users.
  3. The condition of nominating the Award Laureateis to send the completed application form to the organizers.
  4. Application forms are available on the Conferencje website femalesexuality.eu and the European Association for Health Promotion"Pro -Salutem" www.pro-salutem.edu.pl, at the headquarters of the Association or can be sent via post or fax at the request.
  5. The deadline for applications is 28th November 2014, 12:00.
  6. Application forms should be sent via e-mail: farys@indeks-seksualny-kobiety.pl ("Motyl 2014" plus the category should be the message subject), or via post to the following address: Europejskie Stowarzyszenie Promocji Zdrowia"Pro salutem", Marconich Street 3/18, 02-931, Warsaw, Poland, with “Motyl 2014” note.

III. Proceedings/Evaluation

The “Motyl 2014” Laureate in three categories is chosen by the Motyl 2014 Award Jury which consists of:

    Head of the Clinic of Gynecology and Oncologic Gynecology in Warsaw
  2. dr. Ryszard Poręba, MD, PhD
    Polish Gynecological Society, Silesian Branch
  3. dr. Włodzimierz W. Sukiennik, PhD
    1st Vice President of the European Association of the Promotion of Health "Pro-Salutem"
  4. Donata Farys
    Redaction Secretary: „Medicine Courier”, Jury Secretary

The prize is awarded annually on the basis of the jury’s meeting report. Selection of the winner is done in two stages.
In the first stage, the Jury President selects the ten leading Winners for every category on the basis of submitted applications and materials. Jury decides about choosing the Award Laureate through secret voting (the simple majority of votes) in the presence of at least half of the Jury.

The Jury may:

1.concider opinions of independent experts of its choice,
2.decide to withdraw from awarding the Prize in the particular year;

The winner of the Internet voting is the Candidate that receives the highest number of nominations.

If votes are spread equally, the Jury is called together.
 

Jury’s decision will be published on the website of the Conference "Female Sexual Function Index': www.femalesexuality.eu and on the website of the European Association for Health Promotion "Pro -Salutem "www.pro-salutem.edu.pl and in other media and publications.

In order to preserve the independence of the Jury, members undertake to carry out the work related to granting of the Awards for free.

The Prize "MOTYL 2014" is a commemorative diploma and a statuette.

In the subsequent editions, the organizer allows for the possibility of extending number and forms of the awards.
The ceremony of awarding of diplomas and statuettes will take place during The Second International Conference on Female Sexual Function Index – "Intimate Side of Women", 4-5th December 2014, Sofitel Warsaw Victoria Hotel in Warsaw.

IV. Final provisions

The organizer of the annual MOTYL-2014 Awards reserves the right to final interpretation of these Terms and Conditions.
